Please follow the steps below:

1. Contact the Medical Records Department

You may request your records by phone, fax, or email:

2. Complete a Release of Information (ROI) Form
A Release of Information form may be required to process your request. The Medical Records team will let you know if this is needed and help guide you through the steps.

3. Submit Your Request
You can submit your completed ROI form by:
Emailing it to: smdc@sycuanmed.org
Faxing it to: 619-445-0901
Dropping it off in person at Sycuan Health Center
4. Allow Time for Processing
Please allow 5–7 business days for your request to be completed. Processing times may vary depending on the type of records requested.
