How to Request Your Medical Records

We’re happy to assist you with getting your medical or dental records.

Please follow the steps below:

1. Contact the Medical Records Department

You may request your records by phone, fax, or email:

2. Complete a Release of Information (ROI) Form

You may need to fill out a Release of Information (ROI) form to receive your records. If this is needed, the Medical Records team will tell you and help you with the steps. The ROI form is available below.

3. Submit Your Request

You can submit your completed ROI form by:

4. Allow Time for Processing

Please allow 10 business days for your request to be completed. Processing times may vary depending on the type of records requested.
