1. Contact the Medical Records Department

2. Complete a Release of Information (ROI) Form

You may need to fill out a Release of Information (ROI) form to receive your records. If this is needed, the Medical Records team will tell you and help you with the steps. The ROI form is available below.

3. Submit Your Request

You can submit your completed ROI form by:

Dropping it off in person at Sycuan Health Center
4. Allow Time for Processing

Please allow 7-10 business days for your request to be completed. Processing times may vary depending on the type of records requested.
