  • Know the names and roles of your care providers
  • Refuse treatment and be informed of alternatives
  • Access your medical records and request corrections
  • Receive information about advance directives
  • File complaints or grievances without fear of retaliation
  • Expect confidentiality and privacy of your health information
  • Be free from abuse, neglect, or exploitation
  • Change providers when available
  • Decline observation or treatment by students or non-credentialed staff

Patient Responsibilities

As a patient, you are expected to:

  • Provide accurate health and insurance information
  • Follow treatment plans and ask questions when unclear
  • Treat staff and fellow patients with respect
  • Arrive on time or cancel appointments in advance
  • Notify us of changes in contact or coverage
  • Use patient portals responsibly (e.g., MyChart, Intelichart)
  • Participate actively in your care and healing process
